Once you have verified your identity, you will proceed to the online form.

The form can be completed using any device with internet access: any smartphone, an iPad or tablet, a desktop computer or a laptop.

If using the same phone you used to verify your identity, you can just click on the link provided at the end of the process, which will take you to the form.

If using another device, you need to go to this page: Link

You start by providing details of the passport, ID card or residence card you used to verify your identity.

You will then be asked to provide the document details:

Provide your date of birth as shown on your passport:

Verification. You will have the choice to receive either a text or an email with a verification code..

Application type. Once you verify yourself with the code, you will see the main application screen. The sections about identity and photo appear as completed as that was taken care of with the app.

Under application type, you will be asked:

If you have dual nationality. It is possible to enter more than one additional nationality;
If you have a Permanent Residence (PR) document;
If you have been granted Indefinite Leave to Remain (ILR).

Residence. The form will ask whether you have been known by any other names, this is in case there are records under another name. It will also ask for your address and your National Insurance Number.

Criminal convictions. The form will ask questions regarding whether you have ever been convicted of a crime in the UK or abroad, and whether you have been arrested or charged with an offence that you are awaiting trial for.

It is important to answer these questions truthfully. Even if the offence in itself is not a cause for refusal, deliberately lying can be a cause for refusal.
