PR applications for EEA nationals only certify a right that was acquired automatically so they are not immigration applications.

If you have applied for settled and/or pre-settled status, this is regarded as an immigration application.

If you applied for other visas such as visit visa, student, etc. these are also immigration applications that need to be listed.

If you haven't got the exact dates, you can provide an approximate date, as the question says "if known".

The name under which the application was made is your own name, not the name of the application, this is in case you have changed your name since your application, for example, if you got married or changed it by deed poll, you need to provide your maiden name or previous name if that's what you used when the application was made.
